On June 21, 2018, the Charlotte Hornets picked Gilgeous-Alexander with the eleventh overall choice in the 2018 NBA draft, before trading him to the Los Angeles Clippers the same day in exchange for the pick after him (Miles Bridges) and two future second-round picks. He went on to represent the team at the 2018 NBA Summer League, averaging 19 points, 4.8 rebounds, 4 assists, and 2.3 steals per game.

On October 8, 2019, Gilgeous-Alexander made his preseason debut for the Oklahoma City Thunder against the Dallas Mavericks. He had 24 points and four rebounds in a 119-104 victory over the Mavericks.
